Welcome to the Anaheim Ducks i3 Roller Hockey League! We are excited for you to join one of the fastest growing sports in Southern California; ROLLER HOCKEY! Below is a survey and a player code of conduct. This survey will help us create a better league experience moving forward so your opinion matters! Also, there is a code of conduct at the end of the survey. EVERY player must sign the code of conduct in order to play in the upcoming i3 Roller Hockey season. i3 Code Of Conduct

1) Name*

3) No swearing or abusive language on the bench, in the rink, or at your school during any team function.
4) No lashing out any official no matter what the call is. Coaching staff of your respected team will handle all matters pertaining to officiating.
5) Anyone given a penalty will skate directly to the penalty box.
6) Fighting will NOT be tolerated. Fighting will result in an immediate season ending suspension.
7) There will no drinking, smoking, or chewing of tobacco or use of illegal substances at any of THE RINKS facilities, team function, or at your respected school.
8) I will conduct myself in a benefiting manner at all THE RINKS facilities, school, or any location where a team function is being held.
9) Any player or team official who cannot abide by these rules or violates them will be subject to further disciplinary action which is as follows:
a. 1st Strike – Warning received from an employee of the Anaheim Ducks i3 Roller Hockey League.
b. 2nd Strike – 1 game suspension from the i3 Roller Hockey League. NO EXCEPTIONS.
c. 3rd Strike – Suspended for the entirety of the Anaheim Ducks i3 Roller Hockey League season. NO EXCEPTIONS
